Understanding Acute vs Chronic Prescribing in Homeopathy

Homeopaths prescribe remedies within an individualized treatment plan. Although the same remedies may be used, acute and chronic conditions are fundamentally different states—not just in duration, but in their origin, course, and expression across individuals. Recognizing this difference is essential to understanding and treating each case appropriately.
What's the difference?
First, let's define acute vs chronic illness.
ACUTE ILLNESS:
Sudden onset, intense, short-lived (from a few days to a few weeks) and self-limiting (tends to resolve on its own).
e.g.,
Cold, flu, gastric flu, conjunctivitis, stye, cystitis, food poisoning, injury, strain, bruise, animal bite, insect bite, burn, sunburn, shock, fright, sudden loss, etc.
If an acute condition does not resolve, it can move into chronic territory. Especially if conventional medication is used (as it can suppress the symptoms, create side effects and therefore complicate the symptom picture).

CHRONIC ILLNESS:
Gradual onset, it can take years to even become aware of symptoms, long-standing and usually needs holistic care and lifestyle changes to shift.
e.g.,
Arthritis, fibromyalgia, PCOS, endometriosis, migraines, allergies, eczema, heartburn, IBS, inflammatory bowel disease, ADHD, PANDAS, anxiety, depression, ailments from grief, etc.

Very often, acute illnesses are manifestations of an underlying chronic imbalance.
Anything acute that is recurring is likely part of a chronic picture.
Anything acute that is not resolving on its own or with a well-selected acute remedy is likely chronic.
Okay, so what is the difference between ACUTE and CHRONIC prescribing?
ACUTE CARE
Acute care is for acute illnesses. It is for dealing with what is going on TODAY without addressing 1) an underlying issue or 2) a chronic condition. There is no deep dive into the medical history, family history, life events, etc.

What acute care may look like in a homeopathic practice:
At time of booking:
Homeopaths may ask about the main concern to ensure it falls within their scope of practice, or determine whether referral to urgent medical care is more appropriate (or both!)
The consultation:
Homeopaths typically spend 20-45 minutes with a client, depending on the nature of the issue. The focus is on gathering all the information related to the presenting complaint and what has changed since this issue began - across physical, mental and emotional levels.
To individualize symptoms, many homeopaths use some version of the the CLAMS framework:
Concomitant: What other symptoms are present? What else has changed?
Location: Where is the issue located?
Aetiology: When did it start, and what was happening at the time?
Modality: What makes it better or worse?
Sensation: What does it feel like?
In short, the goal is to understand the individual’s unique experience of the symptoms.
Because people experience pain and discomfort differently, homeopaths aim to understand each person’s specific expression of their condition.
Prescription:
Homeopaths may begin analyzing the case during the consultation and finalize the prescription at the end.
In some situations, additional time may be needed to complete the analysis, with a prescription provided shortly afterward (for example, within 20 minutes).
In acute situations, the choice of remedy may also depend on what is readily available to the client (such as a home remedy kit, local pharmacy, or online sources).
A second remedy may sometimes be suggested as an alternative if the first does not bring the expected response.
Follow-up:
Follow-up is more frequent for acutes until improvement is observed. It is common for more than one remedy to be used over time, as the symptom picture can evolve.
CHRONIC CARE
Chronic care addresses everything beyond the truly acute—it includes persistent acute issues, long-standing concerns, and established chronic conditions. Unlike acute illnesses, chronic conditions do not typically resolve on their own; they reflect patterns that have become entrenched over time and require careful attention. Homeopathy is particularly well suited to chronic care, as it aims to address the underlying imbalance contributing to ongoing symptoms. While conventional care may alleviate, palliate, stabilize, or suppress symptoms without addressing this deeper level (the vital force), homeopathic chronic care seeks to stimulate the body’s innate healing response, supporting a more sustained and meaningful direction of healing over time.
Chronic conditions arise when something becomes stuck. By freeing this imbalance, homeopathy helps shift the direction of healing forward.

What chronic care may look like in a homeopathic practice:
The consultation:
Homeopaths typically spend 1.5 - 2 hours with a new client. The focus of the initial consultation is to develop a deep understanding of the client's concerns, history and unique experience in order to see the person as a WHOLE. This allows the homeopath to identify the underlying imbalance and prescribe an individualized treatment plan.
The intention is not only to address the presenting complaint and provide support in the present, but also to work more more deeply - uprooting the issue and supporting long-term health and vitality.
The homeopath must find one remedy (or series of remedies) out of 5,000+ remedies that matches the client's unique suffering. No small task!
During most of the consultation, the homeopath listens carefully, guiding the conversation with thoughtful questions. Toward the end, more specific follow-up questions may be asked to ensure nothing important has been missed. A consultation may begin with a simple question such as, “What is bothering you most?”—with the goal of understanding what is truly impacting the client’s quality of life on any level: mental, emotional, physical or spiritual.
Some questions may seem unusual or unrelated, but they help narrow down the remedy by revealing patterns that are not immediately obvious.
In contrast to acute care—which focuses on a single, recent complaint and what has changed since it began—chronic care consultations explore a much broader and deeper picture. Depending on the individual, this may include:
Presenting complaint
Other complaints
How the client experiences the symptoms (as in an acute but more in detail)
Energy levels
Digestion and appetite
Sleep patterns
Mood and emotional state
Lifestyle factors
Menstruation
Outlook on life
Significant life events
Traumas
Relationships and social environment
Childhood history
Birth history
Anything relevant from the client's intake form (accidents, surgeries, medications, treatments, family history, etc.)
The homeopath is looking to either go deep into one area, or go broad across many areas to find the core issue that needs to be healed. They are looking to find the pattern that connects all of the above.
They are also listening for any obstacles in the way of healing, or maintaining causes - anything external that is keeping the client in their current state (e.g., chronic stressors, environmental exposures).
Unlike acute care, chronic care typically involves an ongoing therapeutic relationship over several months (often at least 3–6 months). For this reason, it is important to work with a homeopath who feels like a good fit.
Prescription:
Like acute care, homeopaths may begin analyzing the case during the consultation and finalize the prescription at the end.
In some cases, additional time may be needed to complete the analysis. The homeopath will then follow up with the prescription within a reasonable timeframe.
A single remedy or a series of remedies may be prescribed. Often these must be ordered which may involve a delay before beginning treatment.
Follow-up:
Follow-ups are generally less frequent than in acute care, as the body needs time to respond to the remedy. Consultations may be every 4-8 weeks, depending on the case and on the client's progress, with e-mail contact in between as needed.
Follow-up is an essential part of chronic care. The remedy, potency, or dosing schedule (posology) may need to be adjusted based on the client's individual response, which can vary significantly from person to person.
